Carolyn Hezekiah Hoitela, MLS

Carolyn has led the Physician Office quality improvement projects for HQSI for over 10 years. She is responsible for project design, implementation, and evaluation. By working directly with physician offices across the state, Carolyn has amassed a great deal of expertise and insight into how physician office practices. She is knowledgeable about physician office quality measures, preventive care, information technology, workflow redesign/strategy and the various government reporting and incentive programs affecting physicians today. As HQSI’s Director of Information Services, she is also responsible for the design and maintenance of all databases in support of HQSI’s organizational goals and contract deliverables.

Carolyn also leads Primary Care Workforce Innovations: Learning Network and Technical Assistance, a non-Medicare project lead by HQSI. This project aims to help primary care practices to redesign how they provide care to be more patient-centered in order to remain competitive in today’s dynamic healthcare environment.

Carolyn has been with HQSI since in 1997; she received her bachelor’s degree in Marketing from the Fashion Institute of Technology in New York and her master’s in Library Science from Rutgers University.
