Form a Team

Patient flow through healthcare delivery is a key factorDownload the Team Roadmapin determining team members. Teams may include:

  • Physician/other clinicians/staff from primary care provider sites (e.g. physician practices, ambulatory care centers, community health centers, or long-term care facilities)
  • Clinical pharmacist(s)
  • Leaders from other healthcare delivery settings (e.g. hospitals, home health agencies, short and long-term care) as appropriate
  • Patients or patient advocates
  • Other members could include health plans, community, and statewide stakeholders (e.g. schools of pharmacy, medicine, and nursing; professional organizations)

Identify a population of Medicare beneficiaries who receive ongoing care by their primary care provider and meet at least one of the following criteria:

  • Take the anticoagulant warfarin on a regular basis
  • Take hypoglycemic medications for diabetes and are not controlled
  • Take antipsychotic medications

Engage in rapid cycle improvement

  • Monitor, track, and report health status and patient safety data for measurable improvement
  • Participate in New Jersey learning and action network, learning sessions, monthly support calls, online discussion groups, and listservs
  • Teams can focus on a single population or on multiple populations

How will HQSI Help?

  • Facilitate and guide teams in their work 
  • Provide on-site technical assistance
  • Assist with project-specific data reporting and fulfillment of PSPC requirements
  • Provide analytical expertise to evaluate and meaningfully interpret Medicare data in your community to help guide activities and interventions and to measure health outcomes, utilization trends, potential cost savings and avoidance
  • Assist with identifying grants and other sources of funding to support the project
